The Burkitt lymphoma (BL) International Prognostic Index (BL-IPI) provides robust discrimination of survival and can be used for prognostication, according to a study published online Jan. 27 in the Journal of Clinical Oncology.
Adam J. Olszewski, M.D., from the Warren Alpert Medical School of Brown University in Providence, Rhode Island, and colleagues derived the BL-IPI from real-word data from adult patients with BL treated with immunochemotherapy in the United States between 2009 and 2018. Candidate variables that showed the strongest prognostic association with progression-free survival (PFS) were identified.
The researchers found that factors with an independent prognostic value identified from the derivation cohort of 633 patients with BL were age ≥40 years, performance status ≥2, serum lactate dehydrogenase >3 times upper limit of normal, and central nervous system involvement. The resulting BL-IPI identified groups with low, intermediate, and high risk (zero, one, and two or more risk factors [18, 36, and 46 percent of patients, respectively]), with three-year PFS estimates of 92, 72, and 53 percent, respectively, and three-year overall survival estimates of 96, 76, and 59 percent, respectively. In the validation cohort of 457 patients, patient characteristics, size of BL-IPI groups, and outcome discrimination were consistent, with three-year PFS estimates of 96, 82, and 63 percent for low-, intermediate-, and high-risk BL-IPIs, respectively.
“This novel, validated index can help clinicians more accurately identify a prognosis for their adult Burkitt lymphoma patients, as well as advance targeted clinical research studies,” Olszewski said in a statement.
